No, the POLST is intended to complement an advanced health care directive, not replace it. An advanced health care directive is still necessary to appoint a legal health care decision maker, and is recommended for all adults, regardless of their health status. If you or your loved one has been diagnosed with a life limiting illness it is recommended that you complete both an advanced health care directive and a POLST form.
