Who provides hospice care?

Hospice care is a comprehensive service which includes nursing care, home health aides, medical social workers, chaplains, volunteers, and of course physician treatment and review.  Most Hospice patients retain their personal primary care physician throughout their time on Hospice which is ideal from the standpoint of continuity of care.
