have been hearing about a legal document for advance health care planning called a POLST form. What is it?

A Physician Orders for Life-Sustaining Treatment (POLST) form is a physician order that helps give seriously ill patients more control over their end of life care. The POLST form documents whether or not a person with a serious illness wants such treatments as CPR, IV antibiotics and fluids, use of a ventilator to help with breathing and artificial nutrition through a feeding tube.
